Understanding Neuropathic Bladder

A المثانة العصبية العصبية (or neurogenic bladder) is a bladder dysfunction caused by a neurological disease or spinal cord injury. The bladder may either fail to store urine (causing leaks) or fail to empty completely (causing urine backup and high kidney pressures).

Causes of Neuropathic Bladder

Neuropathic bladder results from spinal nerve malformations or injuries, including:

  • Spina Bifida (Myelomeningocele): A congenital defect where the spinal cord does not close properly during gestation.
  • Tethered Cord Syndrome: The spinal cord is abnormally stretched or bound.
  • Spinal Trauma or Tumors: Acquired nerve damage affecting the bladder reflex pathways.

Signs of Neuropathic Bladder

Common signs in children include:

  • سلس البول: Continuous dribbling of urine or frequent daytime/nighttime wetting.
  • Difficulty Emptying: Straining or poor urine stream, leaving a full bladder.
  • التهابات المسالك البولية المتكررة: Frequent bladder or kidney infections accompanied by unexplained fevers.
  • Renal Hydronephrosis: Swollen kidneys on ultrasound scans due to high backup pressures.

أهمية التدخل المبكر

A neuropathic bladder can cause abnormally high pressures within the urinary tract, leading to severe kidney damage over time. It can also cause persistent incontinence, which significantly impacts a child's quality of life and social development. Early and proactive management is crucial to protect kidney function, achieve urinary continence, and foster the child's independence.

نهج الرعاية الشاملة

Managing a neuropathic bladder is a lifelong journey. We provide a customized care plan that includes urodynamic testing, medication, and specialized catheterization training. Our team works closely with parents and children to develop a routine that fits their lifestyle, aiming for maximum independence and confidence as the child transitions into adolescence and adulthood.

خيارات العلاج

Medical Management

Use of anticholinergic medications to relax the bladder muscle and lower internal pressures.

Clean Intermittent Catheterization

Training families and children to safely empty the bladder using a catheter to prevent kidney damage.

Bladder Augmentation

A major surgical procedure using a segment of the bowel to enlarge the bladder and reduce pressure.

The Clinical Journey

العملية الجراحية

01. Urodynamic Testing

A vital diagnostic step to measure bladder pressure, capacity, and sphincter function, which dictates the surgical plan.

02. Complex Reconstruction

Dr. Chowdhary performs major reconstructive procedures focused on achieving both kidney safety and social continence.

03. Independence Training

Post-surgery, our specialized nurses work with the family to master the care of the new urinary channel and ensure long-term success.

استفسارات شائعة

الأسئلة الشائعة

What is a neuropathic bladder?

It is a bladder dysfunction caused by nerve damage, common in children with spina bifida, spinal cord injuries, or sacral agenesis.

What are the risks of untreated neuropathic bladder?

Untreated neuropathic bladder can lead to high bladder pressures, kidney damage (hydronephrosis), and recurrent kidney infections.

What is Clean Intermittent Catheterization (CIC)?

CIC is a safe and painless procedure where a small tube is inserted into the bladder regularly to empty it completely, reducing pressure and infection risks.

What medications are used for neuropathic bladder?

Anticholinergic medications are often prescribed to relax the bladder muscle, decrease bladder pressure, and increase capacity.

When is bladder augmentation surgery required?

Augmentation is required if medication and CIC fail to control high bladder pressure, using a piece of bowel to enlarge the bladder.

What is a Mitrofanoff procedure?

It is a surgical creation of a small channel between the bladder and abdominal wall (often using the appendix) for easy catheterization.

Can a child with a neuropathic bladder achieve dryness?

Yes, with a combination of CIC, medication, and timely surgery, most children can achieve complete social dryness and protect their kidneys.
